Health Leads is a Massachusetts-based nonprofit that seeks to improve healthcare for low-income communities. To this end, Health Leads deploys 1,000 college student Advocates to help patients in seven cities access and use various social services. But this new patient care ecosystem only works if what is known about the patient’s issues can be mapped to the right resources and people, quickly. Health Leads built this new ecosystem with an enterprising approach to preventive care, people, and technology.
